  The digital coordination and communication platform for hospital-at-home care went into production on 26 January 2026. The full flow — (1) digital data sharing from the hospital, (2) the feedback on the care provided, and (3) the receipt of questionnaires and questionnaire responses in the care module — was closely monitored throughout the entire week by the whole project team. Minor adjustments will still be needed in the future, but the application is functional. At the same time, tests were carried out for both OPAT and ONCO with a UZ Leuven test patient to fine-tune the operation of the wintermute alerting. The document is written in Dutch.
