Welcome to "Thuishospitalisatie Digitale Eeuw"s Wiki.

Digitalisation of the care pathways for OPAT (outpatient parenteral antimicrobial therapy) and onco@home (home administration of antitumoral therapy). This enables structured communication—covering symptoms, observations, and vital parameters—from primary care or the patient directly to the hospital. The communication is fully standardised and interoperable through the use of internationally recognised standards such as SNOMED CT, LOINC, and FHIR, and is independent of any specific EHR/EVD system or vendor.
