Welcome to Telemonitoring's Wiki.

The Telemonitoring Prescription project (renamed to Transmural Platform - TMP) addresses the lack of standardized telemonitoring integration in Belgiums healthcare system. It aims to create a scalable, interoperable framework that connects hospitals and telemonitoring providers via a unified data integration gateway. This solution reduces hospital workload, enhances remote patient monitoring, and supports the shift to value-based, continuous care.

By implementing three core modules—an Input Module (seamless EHR integration, TMP Portal), an Authentication Module (secure data access), and an Output Module (standardized telemonitoring data exchange, TMP Hub)—the project ensures efficient, secure, and structured telemonitoring. It enables scalable remote monitoring solutions, improving patient care while minimizing redundant IT work.

The project targets integration across at least 10 hospitals, 4 EHR vendors, and 4 telemonitoring providers, with validation on 1,000+ patients. It aligns with Belgian and European healthcare standards (FHIR, HL7) and lays the foundation for future expansion and policy adoption. By standardizing telemonitoring, this initiative will enhance healthcare efficiency, accessibility, and long-term sustainability.
