The project brings together three hospitals, nine residential care centres (WZCs), and the AI vendor Bingli to develop, test, and implement an AI‑based differential diagnostic triage tool tailored to geriatric care. The tool guides nurses through a structured assessment when they sense clinical deterioration in a resident. Based on symptoms and observations, it proposes differential diagnoses and supports the GP or coordinating physician (CRA) in deciding on hospital admission, additional investigations, or on‑site treatment.
By standardising assessments and improving information flow between WZCs, hospitals, and GPs, the tool reduces unnecessary hospitalisations and improves the quality of handovers. A continuous feedback loop strengthens both clinical reasoning and AI performance, supporting more integrated, efficient, and high‑quality geriatric care.
