This project aims to transform geriatric patient care by leveraging innovative technologies and recognized standards: Data Standardization: Use of FHIR and SNOMED CT standards to harmonize care pathways and enhance interoperability across systems.
AI-Driven Clinical Support: Development of predictive models (frailty detection, rehabilitation potential) to assist clinical decision-making and anticipate patient needs. Data Centralization: Creation of an FHIR data warehouse in each hospital populated with data from Electronic Health Records (EHRs) and Patient-Reported Outcome Measures (PROMs), in collaboration with multiple hospitals (Clinique Notre Dame de Grâce, CH Mouscron). Secure Data Exchange and Quality Indicators: Implementation of a FHIR-based data exchange system between hospitals, and generation of clinical quality indicators (such as frailty risk using the SEGA scale) with CQL (Clinical Quality Language). Security and Confidentiality: Ensuring secure data sharing in compliance with regulatory requirements.
This project is part of a continuous effort to improve care by combining technological innovation with clinical expertise.
