Welcome to "Hybride Zorgpad Diabetes" Wiki.
This project aims to fundamentally improve care for people with type 2 diabetes by developing and testing a hybrid care pathway that combines traditional follow‑up with digital support, data sharing, and remote monitoring. Building on existing care structures—home‑nursing trajectories and hospital diabetes conventions—the project introduces connected measurement devices and a smartphone app that allow patients to monitor their health values themselves.
A central care hub (Z-plus) monitors these data in real time, performs triage, and involves the appropriate healthcare professional at the right moment. All relevant information and interventions are shared across care providers, enabling personalised, just‑in‑time advice and timely therapy adjustments. Patients also gain access to a digital knowledge library offering lifestyle guidance.
