Welcome to HEARTwise's Wiki.
Within the HEARTwise project, the aim is to achieve an overall optimisation of the workflow for measuring vital signs and calculating the National Early Warning Score (NEWS), both inside and outside the hospital.
Objectives
- Efficiency and effectiveness through technology: 1) Deploying wearables that automatically record vital parameters and transmit them to a platform that can generate a NEWS score (both intramural and transmural); these parameters and NEWS values are integrated into the EHR. 2) Using AI and developing a machine‑learning model to detect early risks of clinical deterioration and predict worsening of a patient’s condition, enabling healthcare professionals to intervene earlier.
- Extending patient monitoring to the home environment, using NEWS calculations based on wearable‑measured vital signs (supplemented with symptom questionnaires), supported by a digital patient platform integrated with the EHR, enabling close collaboration between the patient and their healthcare providers across care levels.
- Developing and implementing a clinical dashboard for monitoring and risk detection of patients both inside and outside the hospital, supporting clinical decision‑making.
