Welcome to the ERAS@HOME Wiki.
ERAS@HOME is a Data Innovation project focused on integrated, personalized pre‑ and postoperative care for patients in their home environment. The project builds on the international Enhanced Recovery After Surgery (ERAS) model, which aims to achieve faster, safer, and better recovery through a multimodal perioperative care pathway.
Objectives:
- Create a transmural network care pathway that enables patients to be safely, efficiently, and personally followed up at home before and after surgery.
- Successfully implement two concrete pilots: colectomy patients and hemorrhoidectomy patients.
- Promote self‑management and active patient engagement.
- Reduce the length of stay (LOS) in the hospital without compromising safety or comfort.
- Optimize preoperative preparation through prehabilitation.
- Strengthen cooperation between the 0th, 1st, and 2nd lines of care.
- Digitize care pathways in accordance with the most recent ERAS guidelines.
