Welcome to BE Patient Problemlist's Wiki.
The BE Patient Problemlist project aims to enable a hospital‑wide, clinically usable SNOMED CT–coded problem list that can be exchanged nationally via FHIR. The project captures best practices and technical guidance for implementing structured problem lists in different EHR systems, develops benchmarking and quality‑improvement tools, and explores methods for integrating SNOMED CT through local tables or terminology servers.
It also created a cross‑hospital use case allowing problem lists to be viewed, compared, and consolidated across institutions using a dedicated ViewerEditor integrated into multiple EHRs. This includes secure retrieval of external problem lists via COZO, FHIR‑based export and import connectors, and workflows for validating and writing back consolidated diagnoses into local systems. The project collaborated closely with national initiatives (NRC/FOD, BeSafeShare/RIZIV, eHealth, BIHR) and supported broad community building through CMIO networks, whitepapers, and conference presentations.
This hospital‑focused work later expanded into an innovation project involving primary care (BEPL‑Huisarts), with continued coordination across technical, clinical, legal, and organisational stakeholders to ensure safe, standardised SNOMED CT exchange throughout the care continuum.
