Welcome to "Afstemming en Differentiatie van het Zorgaanbod"s Wiki.

This project aimed to integrate innovative, evidencebased digital technology (QIT online) into psychiatric and psychological care. By introducing blended care pathways, the project accelerated and improved screening of mental health difficulties through online psychodiagnostics, enabled systematic monitoring of treatment progress, and strengthened patientcentred care via routine process and outcome measurement. The digital environment also promoted selfmanagement by providing psychoeducation, exercises, and selfhelp modules.

Using a humancentred design approach, clinicians, patients, and their support networks were involved throughout. Blended pathways were mapped, iteratively refined, programmed into the QIT platform, and implemented across three hospitals. Data collected during implementation supported evaluation at both micro and macro levels, providing insights into care quality and opportunities for continued improvement.
